Informed Consent for Medical Care

This document explains, in plain language, what the care you'll receive from PALMED Health Group S.A.S. ("PALMED") involves, its scope and limits, and your rights as a patient — under Colombian Law 23 of 1981 (Code of Medical Ethics), Statutory Law 1751 of 2015 (fundamental right to health), Resolution 13437 of 1991 (Patients' Bill of Rights) and Resolution 2654 of 2019 (telehealth and telemedicine). It is different from the Data Processing Policy (Law 1581 of 2012), which covers how your data is used — this document covers the medical care itself.

1. What is informed consent?

It's your free, voluntary and informed authorization to receive the care PALMED offers. Accepting it means you understood this information, had the chance to ask questions, and agree to receive care under the conditions described here. It's your right, not a formality: you can ask anything you need before accepting, and you can decide not to continue at any point.

2. Services covered by this consent

Home doctor visits, virtual consultations (teleconsultation), home medication administration or IV therapy, and issuance of medical certificates, provided by PALMED physicians registered with Colombia's national health workforce registry (ReTHUS).

3. Scope and limits of home-visit and teleconsultation care

Home-visit care and teleconsultation have a different scope than an in-person evaluation at a facility with diagnostic or emergency equipment (Resolution 2654 of 2019). The physician may order tests, refer you to a healthcare provider, or recommend transfer to an emergency department if your case requires it — they'll explain this to you at the time, and PALMED does not continue managing an emergency through this channel.

PALMED is not an emergency service. If you're having a medical emergency (severe difficulty breathing, chest pain, loss of consciousness, heavy bleeding, among others), call your local emergency line immediately (123 in Colombia) or go to the nearest emergency room.

4. General risks and benefits

As with any medical care, an initial diagnosis may change as your condition evolves or new information comes in, and further evaluation may be needed. The expected benefit is timely medical guidance, and a diagnosis and care plan suited to your case, without having to travel when it isn't strictly necessary. If the service includes medication administration or IV therapy, there's the general risk present in any such procedure (for example, an allergic reaction or local discomfort at the injection site); the clinician will ask about allergy history before administering anything.

6. Confidentiality

Your care information and medical record are confidential and handled under PALMED's Data Processing Policy (Law 1581 of 2012). It is only shared with third parties when there's a legal obligation (for example, mandatory health reporting) or your express authorization.

7. Minors and legal representation

If the care is for a minor, or for someone who cannot give their own consent, it must be given by their parent or legal guardian, who may also exercise the rights described above on their behalf.

8. How is your consent given and recorded?

You can read this full document here before your visit. You accept it by replying ACEPTO ("I ACCEPT") to the WhatsApp message that asks for it before your appointment is confirmed, or by signing the corresponding record if your care is in person. In both cases, PALMED keeps the date, time and channel through which you gave your authorization, as part of your medical record.

9. Withdrawing consent

You can withdraw your consent at any time, even during care, by writing to info@palmedhealth.com or WhatsApp +57 323 357 3211. Withdrawing it doesn't affect care you've already received, and doesn't stop you from requesting care from PALMED in the future.

10. Changes to this document

This document may be updated. The current version will always be available at this same address.
